We recommend that you call your specific insurance plan to determine if we are contracted with you plan. If you are not part of our plan we recommend that you ask your insurance plan for a referral to a physician that does accept your insurance.
For patients that have no insurance and are considered self-pay we ask that you please speak with the practice manager to discuss payment options.
We ask that patients please remember the following:
- Copayments are due at the time of service. This is specific to your insurance plan, and we are under contract to collect the copayment at the time of service.
- We will file an insurance claim on your behalf. Once the claim has been processed by your insurance company any balance is the patient’s responsibility to pay within 30-days of receiving the first statement.
- We accept Visa, MasterCard, Discover, checks, and cash.
- We required a 24-hour notice for any appointment cancellations. If this notice is not adhered to a $50.00 fee will be assessed to the patient’s account.
Patients that are scheduled for a surgical procedure will receive a call from our practice manager to review their surgical benefits, and to discuss what the patient’s financial responsibility is expected to be for their upcoming procedure.
Patients who are pregnant will sit down with our practice manager to review their maternity/obstetric benefits. She will explain our billing process, and discuss the need for a maternity payment plan in accordance with their insurance coverage.
